            <title>Jan 31, A Tough Day at the Office</title>

For those who don't know, I'm in the field of psychiatry. Generally you can't do much harm by talking to people, which is nice compared to ER or surgery where a wrong move could easily kill someone.

But, psychiatry's not free of its risks or tragedies.

I had a patient who had come in a few days earlier. He was a smart guy who had made bad decisions about using drugs and was homeless at the time. He wanted to leave the hospital and there really wasn't any reason to keep him there. Usually people are just in the hospital if they are dangerous to themselves, to other people, or so mentally ill they can't provide for themselves outside the hospital. He definitely didn't meet those criteria and wanted to leave, so there was no reason to keep him there against his will.

We had a short talk before he left. He was asking me how old I was and told me he had also thought about psychiatry. He had pursued psychology and lost his job because of drug abuse. He asked what his chances were of getting into medical school with his past. I told him probably not that great, but that the PhD in psychology might be worth going for. He thought that was a pretty good idea and thanked me for my time.

He seemed happy enough when he left. 

We got the news a couple of days later that he had been found dead. We don't know what happened and probably won't know. It's very likely he had an accidental overdose given his long history of opiate abuse.

It's the first time I've had such direct contact with someone who has died. It's an awful feeling. Looking back, I don't think I really would have done much differently, but it still feels awful.

So, if you're thinking about psychiatry, just understand that every job as a doctor has its dealings with death. This was mine. And it's not great.</description>

I definitely recommend an MCAT prep course so that you can get your best score on the MCAT. 

You'll see this from my reviews, but I think Kaplan's a good choice if you like online materials and practice questions and Princeton review is good is you like one-on-one attention and written materials.

If you like to study more on your own, Gold Standard or ExamKrackers is a great choice.

            <title>Nov 26, Neurology rotation</title>
            <link>http://www.medical-school-insider.com/Medical-School-blog.html#Neurology-rotation</link><guid isPermaLink="false">a90acd5de8df0cfdf2b52376a4ae258c</guid><description>I'm on my neurology rotation now and am enjoying it. I majored in neuroscience in undergrad and it's fun to start remembering those things again here. It's interesting to try to think of where a person's neurologic lesion might be located.

It's different from psychiatry in that there are pretty well specified physiologic reasons for the deficits we see. It's sad sometimes, though, as we often can't do a lot for our patients. Nerves don't regenerate very well and so once the damage is done there's not a lot we can do to reverse it. The main thing is prevention of future events or controlling risk factors (like diabetes or hypercoagulable states for stroke).

So, it's interesting, but sometimes frustrating as there's often not much we do for patients. The main exception to that is for headaches which sometimes have great response to medication and sometimes movement disorders can be well controlled on medication (like essential tremors). Parkinson's disease can be controlled somewhat on carbidopa/levodopa (synthetic dopamine and a dopamine metabolizer antagonist), especially at first, although its response tends to decrease over time.

Overall, I'm enjoying the rotation but also glad that I'm doing psychiatry! I had a very meaningful exchange in the ER on Saturday that reminded me why I'm doing psychiatry. She was going through a lot of very hard things leading to panic attacks at night. I felt that I was able to help her understand that what she was feeling was normal (besides the panic attacks), give her some resources and some medication and breathing techniques to help with the panic attacks. It was great to feel like my intervention made a difference.</description>

I'm excited! While residency is rewarding, it's also pretty taxing at times (as I discussed in my last blog post). Since then I've been putting together resources for my patients so that they have better plans for when they leave the hospital. The Salvation Army has a great resource for people who really want to take control of their lives who have suffered from substance abuse/dependence. It's pretty common to see that in the inpatient psychiatric setting. You can see the details of the program &lt;a href=&quot;http://satruck.org/rehabilitation-program&quot; target=_blank&gt;here&lt;/a&gt;. It's a six month plus program that helps people find work and housing at the end of the program. It's a great resource for anyone who has gotten to the point where substances are running their lives.

Anyway, I feel like I'm contributing something. I'll be leaving the inpatient unit and starting neurology next week (after my vacation). It will be sad in some ways to leave the unit. I've learned a TON there about psychiatry and particularly the meds.

I also just gave a presentation today on antipsychotics, their efficacy and cost. The bigger trials have shown similar efficacy between antipsychotics, including first generation 
(like haldol) and second generation (like zyprexa/olanzapine) medicines. So, we need to think about side effects of the medicine, patient preference and cost to the patient when prescribing. I was surprised and how big the difference was in cost for generic vs name-brand drugs. For example, a 30 day supply of generic zyprexa (olanzapine) costs around $30, but the name brand costs about $600! 

Anyway, a lot of random ideas in this post but that's what I've been doing and thinking about lately.</description>

            <title>Sep 27, Some hard things about psychiatry</title>

One of the toughest things is that our medications are limited. Often the medications we use do not cure illnesses like schizophrenia. It's tough to see wen we try everything we have and a patient still doesn't get much better.

Being in an inpatient hospital is also often difficult. People are being kept there against their will most of the time. I don't like that. Often, however, the medications we use do help and by the end of the hospitalization the patients are thanking us and much better than when they started. That's something I do like about it.

Another things that's been hard to see is that there aren't a lot of places for the mentally ill to go. For people who are chronically ill, families often find that they can't manage their family member at home. These patients then can end up in facilities that are, well, not 5 star hotels and end up staying there for a long time. It's a hard question of what's the best place for these people to be, especially if their illness makes them dangerous to other people.

I guess the main hard thing, which is probably hard for most specialties, is the realization that you can't fix everything. You do your best. Also, it's a little tough to learn that people have to make their own decisions and take control of their own lives. Some patients want the doctors or social workers to do everything for them, which isn't healthy for them or for the people helping them. Learning to step back and help the person take responsibility is a tough lesson to learn.

Overall, though, I definitely still enjoy my job. I wanted to go into psychiatry to help people improve their lives. To improve their jobs and their relationships. &quot;To work and to love,&quot; like Freud said. I feel like often we're able to do that and sometimes we see genuine miracles in how quickly the medicines can help someone go from thinking completely irrationally to rationally once again. 

So, like any specialty there are pros and cons. These are some of both about psychiatry. Hopefully this will help those of you considering a career in the field!</description>

Residency is very busy. After that first great day of residency, I definitely went through some harder times. Sometimes I felt upset that we weren't doing much to help some of our patients. They would be on the unit for a long time, not doing much better no matter what medicines we tried. It was upsetting, but I did learn the importance of talking with my co-interns to share concerns. It definitely made me feel better to know that other people felt the same as I did.

I also had 5 weeks of working nights since I last blogged. That was pretty tough. At my residency, there's only one person on at nights, and that was me! It was pretty crazy to be the only psychiatrist in the building covering everything that happened in the hospital, plus anything that came into the ER, plus any consults that happened in the medical hospital. I had only been in residency for 2 weeks when I was put in there on my own! But, it was definitely a great learning experience and now I feel much more confident in my interviewing, diagnostic and treatment skills. Not totally confident of course, but much more than when I started.

I also had some patients that got better really fast with the medications we prescribed, which was very rewarding.

So, overall, it's still been good, although I'm starting to see the pros and cons of the job.</description>
